Mental Health Professional Player Form

The opportunity to compete in a match play format is offered to our Mental Health Professionals at no cost. All players must have experience in pickleball play and understand the rules of the game.
We encourage you to promote your participation in Pickleball For Pete’s Sake, and solicit donations on your behalf. All donations received in your name will be donated to the non-profit agency you work for. Those mental health professionals working at a for-profit organization are encouraged to raise funds in their names. Those funds will be split evenly among the non-profit agencies participating.
